Kindergarten Registration

The steps below must be completed to proceed with online registration.

Please Note: Uploaded documents must be clear and easy to read to be accepted. Ensure all documents are well-lit, the images are cropped to the edges of the paper (corners to the ends of the screen), and there is minimal background visible. Images that are dark, blurry, or cut off may require resubmission and will delay the registration process.

Step #1: Required Documents

Gather the documents below and scan them to your device. You will be prompted to upload each document during the online registration process. If you skip steps 1 and 2, you will not be able to complete your registration.

Parent/Guardian Proof of Residency

To prove residency in our District, you must provide ONE of the following that show your physical address:

Residential lease or rental agreement
Proof of home ownership (Deed or Mortgage Statement)

If you do not have a lease or deed, you must provide at least TWO of the following that show your physical address:

Pay stub or Income Tax form
Utility or other bills
Voter registration document
Government-issued ID (Driver’s license, permit, or non-driver ID)
Membership documents based on residency (e.g., library card)
Documents issued by federal, state, or local agencies

Student Proof of Age

Proof of age may include:

a certified transcript of a birth certificate or record of baptism (including a certified transcript of a foreign birth certificate or record of baptism) giving the date of birth. Information about replacing a birth certificate is available through the Village and Town Clerks where the birth occurred: https://ulstercountyny.gov/countyclerk/courtrecords.html
Where such documentation is not available, a passport (including a foreign passport) may be used.

If the birth certificate or passport is not available, the District may consider certain other evidence, which has been in existence two years or more. An affidavit of age cannot be accepted as verification. Other evidence may include, but will not be limited to the following:

official driver’s license
state or other government issued identification
school photo identification with date of birth
consulate identification card
hospital or health records
military dependent identification card
documents issued by federal, state, or local agencies (e.g., local social service agency, Office of Refugee Resettlement, etc.)
court orders or other court-issued documents
Native American tribal document
records from non-profit international aid agencies and voluntary agencies

Custody Documentation, if applicable

Step #2: Required Forms

Fill out and save the fillable PDF forms listed below to your own device. You must have these completed files saved and ready to upload during your online registration. If you skip steps 1 and 2, you will not be able to complete your registration.

Important Notes:

The required forms above are fillable when opened on a desktop or laptop. When using a mobile device, you will need to have an Adobe app in order for them to be fillable.

After typing your information into a form, please ensure you save a copy that includes your changes. Before uploading, we recommend opening your saved file one last time to verify that your information is visible.

You will also need to provide the following information during registration.

Parent information - address(es), cell phone numbers, email addresses, and work numbers, if applicable.
Student information - demographic and health/medication information.
Emergency contact information - names, addresses, and phone numbers.
Doctor information - name, practice name, address, phone number, and fax number.
Medical information - diagnosis and medications, if applicable.

Step #3: Additional Information

Submitting the optional documents listed below may expedite the registration process. These documents are required before your child begins school, but will not hold up the registration.

Current Physical
Immunization Records
IEP or 504 Plan, if applicable (special education)
